Phyllis W. Burnett

BURNETT
Phyllis W., age 71, of Apple Valley, MN passed away on July 20th, 2011 at the Augustana Health Care Center of Apple Valley, MN. She is preceded in death by her parents, Earl and Wilma Culver. Phyllis is survived by her daughter, Michelle Duren; 3 grandchildren, Christopher, Susan and Anthony; a brother, Theron (Beverly) Culver; nieces, nephews and friends. Her memorial service will be August 13th, 2011 at 11 AM in the Henry W.Anderson Mortuary, 147th and Garrett Ave., Apple Valley with a gathering of family and friends one hour prior to the service. Interment will held at a later date in the Mt Hope Cemetery, St. James, MN.
